Assessing a casualty

﻿Hello, and welcome to your first lesson in the First Aid unit, Assessing a Casualty. I'm Mr. Clarke and I'm going to be your teacher for today's lesson and I'm really excited to be here with you. And that's because first aid is an extremely important topic because knowing what to do in an emergency situation may lead to you one day, saving someone's life. So come on, let's get started. To be able to participate fully in today's lesson, you're going to need just two things, and that's an exercise book or a piece of paper, and also a pen. So if you haven't got those in front of you now and you need to go and get them, just pause this video, and then press play when you're ready to start. By now you've already started your learning journey for today by taking part in the intro quiz and I look forward to seeing how you did. During the rest of today's lesson, we'll cover the following areas, checking for danger, airways, breathing, and circulation before you'll then be ready to take the exit quiz and I already know that you're going to do great in it. Before we get into the lesson though, there are a few key words that you'll need to know. So let's have a look at them. First of all, we have first aid. Now that means to refer to help given to sick or injured people, until medical treatment is available for that person. The next word is assessing, which means the act of making a judgement or a decision. So when we're assessing a casualty, as this unit is called that means looking at somebody who is injured, our casualty and making a judgement or decision about what first aid that person might need. And finally, we have the word unconscious, and this is when someone is not awake or responding to their environment. Right now, I am conscious and I hope you are too, because we are awake and we are responding to what's happening around us. Whereas if someone is unconscious, that might not be the case. Now we understand what is meant by first aid, I've got your first task of today's lesson. And that is that I would like you to think of any examples of when someone might require first aid and write them down for me. That might, for example, be something like, if they've hurt themselves and they've got a burn or a scald, but I'm not giving you any more clues. I'm sure you've got loads of ideas yourself. So pause this video now, while you complete the task, and press play again, when you're ready to resume. Let's see how many you managed to find, because during that break, I also had a thought too. So let's see if any of our examples match up. Some of the ones that I came up with and you could have had included things such as cuts and grazes, nosebleeds, head injury, sickness, choking and as I mentioned, for my example, burns and scalds. What other ones did you come up with? Before we undergo any actual practical first aid, we must make sure that the environment the person injured is in, our casualty is safe for us to enter. We call this part, checking for danger. On your screen now, there are two locations, a building site and a park. Your task now will be to look at the locations of a building site and a park, and write down what dangers we might need to check for before we go and assist someone with first aid. To give you a clue, I'm going to work with you on an example. Here's a street location, and here are some possible dangers we might need to be aware of and watch out for. For example, if someone was injured and required first aid, I would need to be aware that there isn't any broken glass around, before I walk towards them on knelt beside them. I might also need to make sure that there are no vehicles or cyclists around that might bump into us because then suddenly, we've got two casualties. So pause this video now, and do the same task for the building site and park location. What dangers can you spot or think about that you might need to be aware of? Let's have a look now at the first example of a building site. Some examples for possible dangers might've included live wires, which means potentially you need to make sure that the power is off so you don't get electrocuted. There might be tools or equipment lying around on the floor and depending on the building site, there might also be chemicals that are on the floor which have spilled. In a park, just like the street, we might have broken class which we need to avoid, other people who are around us and might not see us and therefore collide with us, and also any other sharp objects that might be on the floor near where our casualty is lying or sitting. We've checked for danger and we now know it's safe. Whew! So we can approach the person who is injured, and we can administer first aid. You're going to see now a video of me doing the next stage, of assessing a casualty, which is the airway stage. I'm going to give you a little bit of a clue though. You might want to take some notes of the steps I take in the video, because you may or may not need them for a task afterwards. Before checking the airways for my patient I need to check firstly if they're conscious or unconscious. Remember we looked at unconscious as our key word at the start of the lesson. It refers to whether the person or the patient here can respond or react to what's happening around them. We can check this by simply going up to them and asking, hello, can you hear me? It's Mr. Clarke, can you hear me? Another way in which we can check to see if the person's conscious or unconscious unresponsive is by tapping a very gently on the shoulder like this. Hello, can you hear me? At this point, I can see that my patient is unconscious 'cause they're not responsive to either me speaking to them or me tapping on the shoulder. Which is now where we're going to move on to the next point, which is opening up the airway. To the open up the airway, you're going to get one of your hands I'm going to use my left hand, and place it on the forehead of your patient. You're then going to get two fingers from your other hand, and placed them underneath the chin like so. You're going to slowly push your hand backwards on the forehead and the two fingers upwards, and this will move the head up as well. This then opens up the airway and stops the tongue from falling to the back of the throat, potentially causing choking. Now, if we suspect that our patient has incurred a spinal injury, we need to take extra precautions. So we're going to take both of our hands, and we're going to place them either side of the head but whatever you do, do not move their head. You're just going to place them like so. You're then going to use your fingertips, and very gently push the jaw upwards like this. I hope you're watching carefully and you made those notes as I suggested, because I've got a task for you. In a moment I'm going to ask you to pause the video, but not yet. And you're going to see at the bottom some words that will appear. With those words, you're going to need to place them in the missing gaps of this paragraph. If they fall, open their airways by placing a on their forehead, and lifting the tip of their chin with 2, moving the tongue away from the back of the throat. If a, injury is possible, place hands either side of their and use fingertips to lift the angle of the, upwards without moving the head. Pause this video now and write out the paragraph using the words that you see now at the bottom to fill in those blanks. Press play when you're ready to resume and we'll check the answers. Welcome back. How did you find that? Difficult or easy? Okay, well let's find out how you did, shall we? I'm going to read the paragraph again, but this time with the words that were missing now no longer missing in it. Let's see how it should have sounded. If they fall unconscious, open their airway by placing a hand on the forehead and lifting the tip of the chin with two fingers, moving the tongue away from the back of the throat. If a spinal injury is possible, place hands either side of the head and use the fingertips to lift the angle of the jaw upwards without moving the head. How did you do? Tick the ones you got right and any that you didn't, that's okay, just change the answer to the correct answer that you see on the screen now. We've checked the danger and opened up the airway. It's now time for our next stage, which is breathing. Again watch me demonstrate how we do this in the next slide. But don't forget, you might want to take those notes again. At this part of the lesson, I'm now showing you how we check for the breathing. I do this by going down like so but leaving a gap between my ear, and the mouth of the patient who's on the floor. What I'm checking for for around 10 seconds is to see if I can see the chest moving slowly up and down, as you would expect from normal breathing and if I can feel the breath on my ear. You should also be able to hear the patient on the floor breathing. Now, if like my patient is here, breathing normally, not gasping for breath, we would then move on to the recovery position and we'll cover this in a future lesson. Let's see how closely you were listening. You should be checking for breath against your cheek or ear for 10 seconds, true or false? Point now to what you think is the correct answer. Three, two, one, let's find out. Of course, it's true. Well done if you got that right. And if not, just note it down on your notebook so that you remember for next time. So after checking the danger then, oh, what was it? Remind me, what did we do first after danger? Of course, airways, and then? Danger, airways, what was next? Can you remember? Breathing? We've now come to a final part of assessing our casualty, and that's called circulation. I mentioned earlier that we need to check if the breathing is normal. And if our casualty has irregular breathing or is gasping for breath, then that is not counted as normal breathing. This usually happens when the heart stops, meaning that so does the blood flow around the body, which results in what we call cardiac arrest. At the sign of this, or if the patient has stopped breathing we would go into what's called chest compressions as part of something called cardio pulmonary resuscitation or you might've heard it called CPR, which we'll cover in lesson three. Let's test what we just learned, shall we? So if someone is gasping for breath, is it true or is it false that I should put them into something called the recovery position? If someone has gasping for breath, is it true or is it false that I should put them into something called the recovery position? Point to the answer that you think is correct on your screen now. In three, two, one. Let's see what the correct answer was. Of course the answer is false, and this is really important to remember. If your casualty isn't breathing normally, you should begin chest compressions immediately and as I mentioned earlier, we will cover CPR including what chest compressions are in lesson three. I can't believe we've come to the end of our lesson already but before we go, let's do one final recap together. So what was the first thing we need to check when assessing a casualty. Can you remember? Say it out loud in three, two, one. Well done, we need to check for danger. So once we've checked the danger, what's the next thing we needed to do? Can you remember? Again, tell me in three, two, one. Well done if you said ensure that the airway is open, remember a, for airway. So we've checked for danger, we've done airway, what's next? Recovery position

﻿Hello and welcome to your second lesson in the first stage unit, Recovery Position. I'm Mr. Clarke, and I'm going to be your teacher for today's lesson. What a privilege it is to be here with you today. As we've already discovered in the last lesson, learning first aid and knowing what to do in an emergency saves lives. So what are we waiting for? Let's get on with today's lesson. To be able to fully participate in today's lesson, you are going to need an exercise book or piece of paper and a pen. If you need to go and quickly find that before we start pause the video now and press play when you're ready to begin again. By now you've already started your learning journey for today by taking part in the intro quiz. I hope you enjoy completing it as much as I did. During the rest of today's lesson we will cover, what the recovery position is for, give you a step-by-step guide on how you do it, and check our learning in a recap, by which point you'll then be ready to take the exit quiz, which all of you absolutely smashed last lesson. Let's see if we can do it again. Today our key words are first aid, which remember refers to help given to a sick or injured person until medical treatment is available. Unconscious, which is when someone is not awake or responsive to their environment. I'm hoping right now, all of us are conscious. I want to check though, before we go any further in today's lesson, that everybody is conscious and responsive. So I'm going to show you three images of someone laying on the floor. I would like to see if you perhaps already know what somebody put in a recovery position correctly would look like. Pause the screen now and look closely at these images. Decide which one you think is showing somebody in the recovery position. Press play when you decided. Worked it out? Let's see if you are correct. It's picture number, three. Well done if you were correct, and don't worry if you weren't, because by the end of today, you are going to be a recovery position expert, guaranteed. We would put someone into a recovery position if they are unconscious, but are breathing and have no other life-threatening conditions. But, why do we do it? Well, putting someone into the recovery position will keep that airway clear and open. Remember, last lesson we spoke about how important keeping the airway open is. It also ensures that any vomit or fluid won't cause them to chock. Hang on a moment. My mind's gone bit blank. I need your help. Remind me, is it true that unconscious is when someone is awake and is responsive to their surroundings, or is that false? Can you point and tell me if that's true or false? Is it true that unconscious means when someone is awake and responsive or is that, have you decided yet? Point to your screen and tell me then in three, two, one. Of course, it false. Unconscious is when someone is not awake or responding to their environment. If like me, you got a bit confused, make sure you write down the definition for unconscious to help you in the future. I think I better do the same. Should we start learning how we actually do to recovery position then? Well, watch carefully as I go through the first few steps with you. At this point, you might decide to make some notes or you might even want to do it alongside me with someone that's with you now. If you are practising on someone though, remember to treat your casualty with respect and care. The first step to putting someone into a recovery position, if it's safe to do so, put the patient lying on their back like this and kneel beside them. You're then going to take the arm that's closest to you, which is this one here for me and I'm going to place it at a right angle beside him like this. The next step is to get the arm that's furthest away from you, bring it across the body and put the palm onto their cheek. The final step for this video is you're going to get the leg that's furthest away from you, and you're going to lift it up and bring it into a right angle and again, hold it there. Did you get all that? Well before we move on, let's have a quick test. Look at the instructions that are on your screen now. The only problem is that they are not in the correct order. Your task in a moment is to write them down, but in the correct order. Pause the screen now to complete the task. When you're finished, press play again to resume and we'll see if you were right. Let's check our order and see if we've got them right. Remember, if you got any wrong, don't worry about it. Just correct your order, so you remember for next time. Can you point and tell me then which instruction is step one? Amazing, and now which one's step two? Of course, and step three? Great. Which means that this must be step four. How did you do? Quickly check and make sure you've got the same that I have. Should we learn the next steps now? Again, at this point you might decide to make some notes or if possible, do it alongside me with someone. But if you are practising or someone, we must always remember to treat our casualty with respect and care. Are you ready for the next steps? So still holding the arm in place there what you're now going to do in the moment is you're going to pull on this knee and put it towards you. In doing that action this will allow the patient to roll over on the side of which you're pulling them towards. Now you don't worry about their head because their head will be supported by their arm and that'll stop them rolling by the right arm that you've created in the first step. So let me show you what happens. So I roll him over, and the patient is now like this. The final thing we're going to do here is make sure that the leg is at a nice right angle, and that the airway is open. We do this by simply lifting up the head as we learned in the first lesson. At this point, we're going to stay with our patient until help arrives. While I was writing down the instructions for step five to nine, I got a little bit confused and I think I wrote down the wrong instructions. Can you spot which two instructions are not part of steps five to nine. Pause the screen and when you found them, press play to resume. Point to the screen of the two incorrect answers found. Are you ready? Point to one now. Oh no, I must have put my tea order in there. Much better though. Of course there were two, was there? So where's the other one. Can you point to the other one for me? Oh, I see, I put them down to lie on their front, which is actually not even in this section and then it shouldn't be on their front. How should they be lying? What's that first step again? They should lie on their back. Thank you everyone. Before I think we can call ourselves recovery position experts, I think it's time for a recap and knowledge check. So let's go through the steps together. As we do this, say them along with me as well. So first of all, we put the person on their back and kneel by their side. Next? Next we put the arm nearest you at a right angle to their body with the palm facing up. Then? Fold the other arm so that the hand touches the cheek nearest you and hold it in place. Use your free hand to bend the knee furthest from you into a right angle, how have you done so far? Well, okay, do you remember the next step? Pull on the bent knee to roll the person over onto their side. Then their arm bent should support their head and their extended arm should stop you from rolling them too far 'cause that wouldn't be good. At this point, we must make sure that their bent leg is at a right angle. Then, what must we make sure is open and it begins with a? We must make sure that, the airways. Open the airway by gently tilting the head back and lifting the chin, checking nothing is blocking the airway. Well done today, you've been absolutely amazing. How awesome is it to think that you might one day be able to potentially save someone's life because of what you've learned in this unit so far? Next lesson we'll learn about something called CPR. Over the last two lessons, we've learned about how to assess a casualty, put someone to recovery position and today we are exploring what it is and how to do CPR. And if you're like me right now, I bet you can't wait to get started. So shall we? Come on then. Cardiopulmonary resuscitation. Well done. And this is an emergency lifesaving procedure performed when the heart stops beating. Have I not been telling you that you are learning lifesaving skills in this unit? Our final two key words today are respiratory arrest and cardiac arrest. Now, respiratory arrest is used when someone stops breathing whereas cardiac arrest is when the heart has stopped. Try to remember which way round it means. So cardiac means heart and respiratory is referring to breathing. Now we know what CPR means. Do you think you can tell me the answer to this question? Pause your screen now to write down your answer and when you think you've got it press play again. Okay, welcome back. Are you ready to find out? Well, you must never do CPR on someone if they're awake and breathing normally. We looked at type of breathing and what wouldn't count as normal breathing in lesson one. So did you get this correct answer? If not, make sure you write it down because it's really important that we remember. Never do this on someone who is awake and breathing. Look at these facts on your screen. Can you point to the one that you think is not true? Can you? I'll give you a countdown. Get ready, read them carefully cause I want you to point at the one you think is not true in three, two, one. Point now the answer you think is not true. Should we see if you're right? The correct answer is that children are not allowed to do CPR. That's because anyone can perform CPR. Although it's sometimes is harder for young people because of the strength it uses up but we'll cover what else you can do if you don't want to do or can't do CPR later on. We must also recognise that at this point that although CPR does save lives, it's not a 100% guarantee and sometimes people still can unfortunately die. Even though CPR might've been performed but there is always a chance that you might save a life through using CPR. CPR is fairly simple. And the two most important aspects of it are called rescue breath and chest compressions. A rescue breath is an artificial breath which is delivered through the mouth via your own mouth usually as you can see in the image below. You might've heard it sometimes being called mouth to mouth. Now, chest compressions you can do via your hands interlocked like this. One hand, place your other hand on top like this. Can you do that? So one hand and the other hand on top. Great. Now, you deliver these to the centre of the chest and you'll see me doing this more clearly in the step-by-step video shortly but can you find the centre of your chest and you go down there's a squichy bit. Can you feel it? Just there. Can you notice how in the images not doing this to someone who's real and that's because we must remember, as we noticed earlier, that we don't do this on anybody who's awake and breathing normally. Now that we've gained understanding of the purpose of CPR, shall we get ourselves ready to see how it's done? Okay. But, just before we get there, can you remember the steps we would take before we get to this point? Pause the video and write down what the four things are that we would check or do before we perform CPR. Press play when you're ready. Are you ready to see how well you did? All right. What the first step. Beginning with D it's danger. Well done. We must remember to check the danger and then we would that the airway is open. Good. To allow us to check the breathing which if we believe is normal we will place them in a recovery position remember from lesson two. And if by doing free circulation, we notice signs of cardiac or respiratory arrest we would go into CPR straight away. During the CPR demonstration video I'm about to show you, I want you to be listening extra closely to try and keep a tally of how many times I say the following words: rescue breath, chest compressions, cardiac arrest, respiratory arrest and CPR. Do you think you're up for the challenge? Do you think you'll be able to keep up? Pause the video now so you can write these words down and press play when you're ready. When you perform CPR there's a little bit of choice involved because if you don't feel comfortable or you haven't been trained to use rescue breaths, then you can perform what's called hands only CPR. This is done by placing the hail of your hand on the breastbone at the centre of the person's chest. Place your other hand on top of your first-hand and interlock your fingers. Position yourself with your shoulders above your hands. Using your body weight, not just your arms, press straight down by five to six centimetres or two to 2.5 inches on their chest. Keeping your hands on their chest, release the compression and allow the chest to return to the original position. Now you need to repeat these chest compressions at a rate of a hundred to 120 times a minute until an ambulance arrives for you or you become exhausted. Now, if you don't remember these steps that's okay. Because when you call an ambulance, the telephone systems now exist that can give you basic lifesaving experience including advice how to do CPR. If you have been trained in CPR, including rescue breaths, and you feel competent using this skill, then you should give chest compressions with the rescue breaths. Now how this works is very similar but apart from with adults you're going to start by giving 30 compressions to begin with. After you've given 30 chest compressions, you'll then going to give two rescue breasts and by tilting the casualty's head gently and lift their chin up with two fingers. You pinch the person's nose, seal your mouth over their mouth and blow steady and firmly into the mouth for about one second. Check that the chest rises and then go and give the other rescue breath. If you are performing CPR to children and you are using rescue breaths as well, then you would start with an initial five rescue breaths. You would then go on to give 30 chest compressions. But with a child, you are likely to only use one hand not two like you do on an adult. So after with a child you've given five rescue breaths and you've given 30 chest compressions, you would then go and give two rescue breaths and repeat the cycle of 30 chest compressions to two rescue breaths. How did you find that? And how closely were you listening to what I was saying? I'm now going to show you how many times I said each of those words and that see if they match up. Here we go. How did you do? CPR is exhausting and very physically demanding that sometimes we may not feel able to do it or indeed feel comfortable doing it. Nicky here is not feeling confident or strong enough to perform CPR. What do you think he could still do to help? I want you to pause this video and jot down some ideas on what Nicky could do. when you think you've got some ideas, press play again. Let's explore some of these examples. And if I got any that you didn't make sure you write them down so you can remember them for next time. He could recognise an emergency, call 999 and look out for the ambulance. He might be able to call out for help from someone. He might also be able to help someone else to perform CPR. He could talk them through the steps, count the chest compressions or the breaths for them, provide encouragement and reassurance that they are doing the right things. So even though Nicky might not have felt strong enough or competent enough to perform CPR himself, there is still so much more he can do and he is still playing a vital life saving role. Before we finish our lesson today, let's do a quick quiz. Write down and match up the statement with the correct number. Pause this video now and press play when you are finished. I'm Mr. Clarke, and I'm your teacher for today and I've really enjoyed exploring and learning life-saving skills together over the past few lessons. From those first steps on assessing a casualty to learning how to do the recovery position and performing CPR. So, although it's our final lesson together I'm still really excited to get going and continue our first aid learning journey. Are you ready too? Well, let's get started then. During the rest of today's lesson we'll be looking at how we find AEDs, understanding how they actually work, and how we use them. This will then give you the knowledge you need to go and complete your final exit quiz of the unit. This lesson today brings us some more key words to explore. So you might like to write these down because they are going to be used through out today's lesson. The first one of course is going to be AED, which stands for automated external defibrillator. And that is a name given to a portable electronic machine that can detect the abnormal heart rhythms that are present when someone has a cardiac arrest. Now, I'm sure you know lots about heart beats and heart rhythms for your PE or science lessons. And I'm sure some of you have even felt what your normal heart rate feels like. When somebody doesn't have that normal heart rate it's called an abnormal heart rate. And that's when we might need an AED. Now, we've seen the word cardiac arrest before in the last lesson but just to remind you cardiac arrest is what we say When the heart has stopped. Over this unit we have explored so much and covered a number of key words. How many of them can you remember though? In a moment I want you to draw yourself a mind map, just like I have. I want you to write down all the key words that you can remember. I didn't do it throughout the lesson as well. At the very end, we'll go back and we'll compare our mind maps, and to help you I've started mine already. On mine I've put AED and cardiac arrest. So pause this video now and complete the task. Welcome back. Can you point to locations that you think are correct. I'm not sure if they're all correct or maybe one or two are correct. Who knows, let's see. Pointing out the answer you think in three, two, one. Interesting. Well, should I let you into a little bit of a secret? They're all correct. You can find AEDs in public places, such as offices, schools, gyms, shopping centres, supermarket, stations, airports, the list goes on, and you can even find them when you're on holiday as well. You just need to look out for an AED signage which you can see on those machines there. And if you can't find one, well, when you ring 999 they'll be able to tell you where you can find one. In some places though, AEDs are kept in an unlocked cabinets. So anybody can access them. Sometimes though, like we see in the phone box the cabinet might be locked. So when you phone up 999, the emergency service operator will give you a code over the phone which will then allow you to unlock it. If possible though, and this is really important. One person should perform CPR, which we covered in what lesson? One, two, or three. What lesson was it? Can you remember? In lesson three. While another person goes and finds an AED? How AEDs work is that when an abnormal rhythm is detected the machine delivers an electric charge, that shock that might be able to restart the heart or establish a regular rhythm. The sooner the heart is restarted then the better the casualty chances of survival is, because it can take time for an ambulance to arrive. So, using an AED could save a life. And that's the important thing to remember. Do you know if true or false that you can only use an AED if you are medically trained to do so? Is that true or is that false? Point to what you think is the correct answer. In three, you're ready? Two, one. What'd you think? Should we say the correct answer? It's false. You must remember that you don't need to be a medical professional or first aider. The machine will speak the instructions for you. And it quite literally guides you through the whole process. Let's recap some things to remember when using an AED. The first one, you cannot harm the casualty because it will only work when it detects an irregular heart rhythm. Some people are really worried about using one in case it's not the right time. Don't worry about it. The machine knows when it's the right time. If possible one person should perform CPR while another finds an AED. Every second or minute that goes by means the casualty's life is on the line. So the quicker we can start the process performing CPR and slash or AED the better. And finally, you are always better off using an AED if someone is having a cardiac arrest than doing nothing. The whole purpose of first aid is to save life. Okay then. How are you doing with your mind map that we started at the very start of today's lesson?
